Why not? You can blood hunt in nonhuman land provinces, why not underwater. (Of course you get human blood slaves from lizard provinces...)

The problem is you can't blood hunt underwater. At least I was never able to do so with for example tartarians.

i've seen this bug before. Haven't checked it latest patch;
you can't blood hunt from the menu of commands from spacebar.
But the shift-B command to bloodhunt does work.
(or whatever the normal bloodhunt command is, i aint' played in awhile.)

Do you actually collect blood slaves? Seems like I remember bloodhunting underwater but never getting any blood slaves...

They might not advertise it, but like the (MA) Kings of the Deep, the Kings of Rain can bring humans beneath the wave. In this particular case, only ten or less.

I suspect this has something to do with this, though I note Cuxtal has 11 blood slaves. Perhaps some quasi realm between gem and unit?
