Wow!

Gaap the ice devil, heroic ability tough skin, went into a battle equipped only w/ a pendant of luck. My army routed but he was berserk and stuck around.

He killed 5 jotun commanders and 58 heavy infantry and archers all by himself!

His HP seemed to be regenerating every round. Berzerk says nothing about this and he does not seem to have regeneration.

Does berzerk recover HPs???

Hellsword has life drain ability which gives wielder HP (even over his max).

Berzerk doesn't give regeneration and it doesn't recover HP.

lets look at it: ice devils are already tough
tough skin makes him tougher
berserk makes him tougher again
most likely fighting in cold dominion made him tougher (you were fighting jot so...)
pendant of luck practically doubles his survivability
hellsword drained the life of all he encountered (its also what gave him berserk iirc)
