If she was sitting in a castle, I'd wager for Vengeance of the Dead. Some mage letting all the poor souls your heroine had killed a chance to get even with her.
As to many things in this game, you couldn't have said it better. And the devil's in the details, for instance I hadn't noticed the spell description of Manifestation doesn't specify that Ashen Angel would attack _enemy_ commanders - until I just botched a castle storming by assasinating my own mages.
Gotta love the game.
