Three or four misc slots. As longas you can put one item between two identical items, everything stacks. Thus, with four slots, you could put 2 eyes of the void and 2 spell focuses, alternating, and it would all stack. Put in a Rune Smasher for good measure and it starts being really painful...

this is the major reason i like the Crone and the fountains. By the way the fountains can use extra eyes and they get no real affliction from it.
