Seeking weapons (including cap. missiles) have a 100% hit rate. As you say, the only time they don't hit is if the target moves out of range, or if they shot down by point defence. Or if the target gets destroyed before the missiles arrive.

In your case, if your ship wasn't moving and didn't have point defence... well that's a bit odd. Maybe it hit the ship but didn't display the explosion. Is the ship damaged? Was it damaged before?

Also, what Version & patch of SEIV are you using? I think some Versions had a bug where ships would fire seekers even though they were out of range.
