If killed, are elementals gone?
After doing a search, I did not find a posting on this and, as I don't have the manual on me, I'm not sure if it is included there but I have a question.
I know that there are a limited amount of elemental summons available and, if you try to summon an elemental which has already been summoned to the maximum amount, you end up summoning no elemental at all.
But what happens if one, or all, of those elementals has been killed off? Can they be summoned again or are they out of the game entirely?

They can be summoned again. Same exact ones. So with eventual horror marks, afflictions etc [well, not items].

Just in case: this only governs elemental royalty (Air Queens, Earth Kings...) and other unique summons, almost all of which are commanders. Normal elementals aren't affected.

Back in the days, the game got confused if the royalty was alive, but mind
enslaved, 'rebirthed', etc... Anyone tried it recently?

I think it's based on unit type, so Ritual of Rebirth shouldn't matter... It'd check if any instances of unittype "Queen of Storms" appear, and if not, summon that one.
Only the King of Banefires can cast Twiceborn, though. Or if you meant Ritual of Rebirth, then that'd probably work, unless it was limited not to affect uniques...

Especially a pillar of fire that's been snuffed out?
I guess you could gather the ashes of where it last stood and put them into an urn, then put the urn inside a mummified body...
But the water and air Queens even more bewildering...

There aren't any uniques that should be allowed to be mummified, right? That'd be like creating the corrupted King of Banefires.

What happens when an unique summon gets killed and is then summoned again by two players simultaneously, how the game decides who gets it? Is it 50/50 chance or is there some other game mechanic for it?

Spell order is random. So you need luck. One player gets elemental, another wastes gems.
