I lost a unit by handing over his waterbreathing item and leaving the seas back in about 1.15, but I can't remember the unit and circumstances. I've avoided it ever since, so this useful to know, thanks.

I did lose my commander in a game of current version by using this trick. I can not remember the detail very clearly, but I think he was trying to invade hostile province, that could be the reason.
