This happened to me a few times in 3.17. Hasn't happened after upgrading to 3.20.
But it is happening in 3.20 to a player in a game I'm hosting.

There's a bug with the file names. The map file refers to "hyboria.tga", but the file provided in the zip is "Hyboria.tga". Which makes the map fail on case-sensitive filesystems.
