Default Under Siege, but no break siege option

Last message of a turn is I am undersiege but the enemy cannot harm my walls.

However, I have no break siege order. Further, I see no siege effects.

How could this have come about? The only way I can think of would be for a spy to siege and then sneak away?

perhaps you were under siege and then the unit disappeared somehow? merc or disease or wished away?

I've had this happen several times in my games, and it's as Archae says in my experience. I always get it from when a thug has raided one of my fort provinces, and I kill it off in the magic phase with an assassination spell of some sort, leaving the fort siege free, but without me ever having A) moved troops into the province, or B) used the 'break siege' option.

Maybe the 'under siege' message is created if you have a fort under siege at teh start of a turn, but neither A or B has happened that turn. As perhaps those are the only ways of removing the 'under siege' status that the siege report mechanic recognises as ways of removing sieges, and does not pick up sieges that are broken by magic spells. And maybe I'll also guess that C) enemy moves troops off fort, is another one the siege mechanic recognises.

I killed no units.
Turn A: Not Sieged. Three enemy spies killed.
Turn B: Sieged.
Turn B Order phase: No Break siege option. No siege restrictions.

You also get the "sieged" message without actually being sieged if you (or someone else) dom kills a nation that is sieging one of your forts. Do you have PD (that you didn't purchase in the current turn) in the province?
