No need to reload. It's a bug.
I have noticed it usually manifests itself when you are trying to colonize a planet in a system you don't currently have a ship in or another colony. It's like your colony ship thinks maybe the planet isn't there if you don't have an actual view of the planet currently.
Although sometimes I have sent colony ships like that and it worked. It's not consistant as far as I can determine.
