Is tehre a chance to fail assasinations attempt ?

1) Using Abysia somtimes happens to me to giva an assasination orders but it seems to have no effect ? Is that a bug ? Is there a chance that the attempt doesn't take place or simply it must be that there are non leaders in the province ?

2) make sense to make an assasination attempts in a province with no units but only national defence ? or killing the leader didn't destroy the defence force ?
