I belive for Crystaline Armour it is for every shot that hits the armour. For Organic I think it only 'heals' after each combat turn.
(These are quite complex subjects if you wish to know exactly...

Just in case the problem crops up again are you using win 95/98? If so and the hang happens again it could be .bmp file headers, there is an known issue with the headers of some .bmp files...

I was thinking that minesweepers might only remove the Tag if they try to sweep a mine and don't find one. This would explain why some times they don't remove the Tag, if for example the ship removes...
