Failing in creating objects

I am rather new here but have never encountered this!

Trying to create The Flailing Hands, a death object, had 2 mages failing simultaneously in last turn. WTF? anyone knows why? nothing in the description of the item saying that the chances of failing are higher etc. Any idea?

EDIT: I actually gave orders to construct to 3 mages, one was successfull... is this an artifact?
