Hmm, just a shot at the dark. What kind of warheads did your mines have? Some components (scatter armor III for example) cannot be destroyed by warheads I. Those ships would plow right through.

edit: also, if you play simultaneous move, sometimes the message that a mine field was triggered is not displayed. I think that happens when the whole minefield is lost, but I am not 100% sure.
