Default Strange warp points question

I'm in the middle of a TDM game (newest Version) against the comp and some strange things are happening with the warp points: My ships seem to be taking the warp points and ending up in systems to which I never directed them. For example, I'll tell a ship to go to a particular planet by warping through a jump point. It will indeed go to that jump point but it actually warps through a different jump point that's in the same system. What's going on?

The other thing I'm noticing is that some of these weird warp holes have the green square around them as if they were waypoint markers (which they're not--I certainly didn't set them up) and have a letter "M" in them at the bottom right corner. But it's not only these kinds of warp points that are acting this way--I THINK.

Now the strangest thing is that I've just now had one of my ships suddenly warp into a system several systems away from my empire's borders--it hit a minefield and died. How on earth did my ship get there? I hadn't sent it anywhere near this system it ended up in!

Again, what could possibly be happening here?
