It raises water magic by one. That's what I'm using it for, mostly.
It has the "Soul Slay" effect when hitting, that's how it can kill in one hit.
No, it doesn't make you insane at all.

The Soul Slay effect is probably the same as for Ethereal Crossbow, so successful magic resistance roll saves against the insta-death.
