I dunno, it might ... probably the best way to check is to run a bunch of tests casting leprosy on a mix of regenerating and non-regenerating units with identical MR and count how many get diseased.

My guess would be that regeneration simply interacts with the actual affliction mechanic, from damage. Leprosy, being a spell that causes a particular effect, that just happens to be classified as an affliction, is probably unaffected by regeneration.

I've seen a nearly dead from disease Marverni druid with a Nature blessing survive several extra months after an assassination attempt against him, so I think the answer is "in battle".

Ah but that's because he only has regen in combat, due to only being blessed in combat. Put a shroud on him and you get a different story.
