Well, I just re-watched the miniseries this weekend... and 12 Cylon models appears twice. The first time, Number Six tells Baltar that there are only 12. The second time, Commander Adama sees a printout note in his office that says something like

Curiously, he notices this just after President Roslin leaves... and then the miniseries ended. And it still hasn't really been answered who exactly left the note (up to the end of Season 1)...
