The sacrificing priests re-equip themselves with bloodslaves as long as you have some in your global 'gems pool'.
If you use all your remaining blood slaves in, say, blood summons, they WON'T re-equip themselves because it will be NO SLAVES LEFT in the pool, since the slaves blood-hunted this turn are in the inventory of the hunters (until you Z them > put all your slaves in the pool).
Edit: they lose the 'perform blood sacrifice' order.
If there is not enough slaves left for all your sacrificing priests, the game will fill them in (ramdom?)some order, and left the lasts with no slaves.
Edit: the last filled priest may have less slaves than previously (those whose's left in the bottom of the pool), so, his sacrifice may be less performant (1 spread check by sacrified slave).
For the afflictions, I never see something like that : always a reason : age, spell, battle, magic artifact ... I don't know if it exists a magical site with an effect like this.
