Okay, well that's odd. Its the long fuselage UH-1/Bell 205 type helicopter icon, and is Icon 748 in Icon0071.shp for me and is the same in the backup .shps I have because I have edited my .shps (something I checked to make sure even though I haven't edited Icno0071.shp).
Are you saying that when you look at Icon 748 its only a four blade rotor, or that it has a four blade rotor? It should still have a two blade rotor unless the icon has changed to something different.

I'm saying that Icon shows four blades in the game, not two, and not three which is what you reported you saw

Also, when I open Icon0071.shp in SHPEdit, the rotors aren't shown in sequence with the helo icons (and I don't see them even when I manually increase the location number). So I'm not entirely clear where the rotor "turrets" for the icons in that .shp are either. Is there something wrong with my copy of the .shp and my backup? I could've backed up a bogus one along the way I guess, not out of the realm of possibility.

The rotors are not showing in sequence there because that file is set up in the code with the blades in a separate file ( Icon0008.shp to be exact ) some shp files were set up specifically for helos and were given the "turret" ( blades) from a file that already had them. When faced with installing hundreds of Icons this was a quicker way to do things.
All of this is obvious when viewing the Icon list found in the Game Data\Design folder . All "UH-1D/H" type Helos have a second number string of "843" ( that's a twin rotor ) but that Icon shows "840" ( that's the four blade rotor ) and this case it's a minor error that's been there since that file was first built and has now been corrected in code and will show up with the "proper" twin blades in the next patch
