Default Command Structure Breakage

I was attempting to put together a small campaign and noticed during my second test, the command structure is completely messed up.

I have altered the USA OOB file to include 5 new formations in slots 410, 411, 413, 414 and 416. These were empty slots so I figured I could use them to make units I wanted included.

I include only one of the new formations in a scenrio, it is in slot B since Slot A is reserved for the Headquarters unit. The other units where two US Mech. Infantry companies, an Armor company, Mineclearing Tank Section, 2 Fist-V vehicles, a Battery of M109A-Paladin's and finally a MLRS section.

After putting together the scenrio, then putting it in the campaign, the command structure skewed as follows:
A)Headquarters
There is no unit listed as being commanded by this unit
B) HHC-Mech Inf (the new formation I put together)
C) Company HQ (Mech Inf)
D) Mech Inf Platoon ( Should be commanded by C but is not)
3 Mech Inf Platoons from Second Mech Company
J) Company HQ (Mech. Inf)
M) Mech Inf. Platoon ( Should be commanded by J but is not)
3 Mech Inf Platoons from first Mech Company

And the rest of the units command is as scrambled as the examples here. Is there something I'm doing wrong?

I've not cross attached any unit what so ever, so how come the command structure is messed up?

Oh, I almost forgot, I set the campaign to have a fixed core structure, would it be better to change this preference and if so, what purpose does the fixed core option serve?
