1) I've heard it was random. Though it may also be the ones furthest to the top (On the army placement grid).

3) I believe the latter (multiple random transformations).

4a) Yes, it probably should avoid it. The best probable solution is that each nation should have a list of globals that it should cast, or the converse, a list of never-cast globals.
